Retired Identification Card Request Process
The following procedures below are set forth for all Louisiana State Police commissioned personnel, who are preparing to retire from full time active duty
or for any retiree that has already retired from full time active duty status, to use when requesting the issuance or replacement of a Louisiana State
Police Retired Identification Card.
- Complete attached Retired Identification Card application.
- Affix 2" X 2" color photograph to application in the indicated box at bottom of page.
- Submit application to Internal Affairs for processing.
- Internal Affairs will confirm dates of employment and provide a report to the Superintendent for review.
- If request is approved by the Superintendent, Internal Affairs will forward to Concealed Handguns for issuance of Retired ID card.
If request is denied Internal Affairs will provide notification to individual requesting ID card and maintain file of such notification.
