Crash Investigations
This agency is a statutorily mandated, statewide law enforcement agency originally instituted to enforce the laws of Louisiana relating to motor vehicles and their operation on the streets and highways of the state. The Louisiana State Police has primary enforcement and crash investigation responsibilities on all state and U.S. highways.
If you are involved in a crash and you or the other parties wish to file a crash report, contact either the nearest State Police Troop by dialing *LSP (*577) or the local law enforcement agency by dialing 911. The appropriate police agency will then be dispatched to you.
If a Trooper is assigned to investigate your crash, at the end of his investigation, he will issue a form by which you may request a copy of the crash report. (If a deputy or officer investigates your crash, then you will need to follow that agency's procedures to acquire a copy of the crash report.)
If by chance you are not issued the crash report request form, you may obtain one below.

Copies of crash reports are available in person at any Troop
or at the State Police Traffic Records Unit at Headquarters in Baton Rouge. If you
want a copy of the report via mail, complete Section I of the form
and mail it to the State Police Traffic Records Unit address below.
A certified check, company check, or money order for $7.50 must accompany
the request. Personal checks and cash will not be accepted.
Please allow ten (10) working days after the crash before submitting a request
for a crash report.
